What Happened

Complaint investigation substantiated two critical violations: partially used infant formula bottles periodically saved and refed to infants (creating bacterial growth hazard), and infants placed on backs with bottles despite inability to support independently (choking hazard).

Observations

  • Partially used prepared infant formula bottles periodically saved and refed/offered later in same day
  • CDC guidance not followed - prepared formula should be discarded after feeding; use within 2 hours of preparation and 1 hour from feeding start
  • Infants placed on backs and provided bottles when unable to support independently
  • Infants not placed in high chairs or appropriate seating equipment during feeding
  • Staff not holding infants unable to sit unassisted during feeding

101427(n) Bottles and nipples maintained by the infant care center shall be sterilized, This requirement is not met as evidenced by: Based on investigation interviews, staff are washing infant bottles in classroom sink with dishwashing detergent but not sterilizing as required which poses/posed a potential health, safety or personal rights risk to children in care.

101239(e) Faucets used by children for personal care shall deliver hot water. This requirement was not met as evidenced by: Based on observation and interviews, infant room sinks does not deliver hot which poses/posed a potential health, safety or personal rights risk to children in care.

**THIS IS AN AMENDED REPORT DELIVERED ON 10/8/24**101227(a) (19) All food shall be protected against contamination. Contaminated food shall be discarded immediately. This requirement is not met as evidenced by: Based on investigation interviews, partially used prepared infant formula bottles have periodically been saved and refed/offered later in the day. Per CDC, "Throw out any infant formula that is left in the bottle after feeding your baby. The combination of infant formula and your baby’s saliva can cause bacteria to grow." By feeding left over prepared formula this poses/posed a potential health, safety or personal rights risk to children in care.

Type B101439(g)

101439(g) Furniture shall be maintained in good repair and safe condition. This requirement is not met as evidenced by: Based on observation, LPA observed both diaper changing table vinyl cushions were torn with foam filling exposed which poses/posed a potential health, safety or personal rights risk to children in care.

Type B101427(i)

101427(i) High chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding.This requirement is not met as evidenced by: Based on investigation interviews, infant who can hold their own bottles should be placed in appropriate seating equipment to feed but are often placed on their backs and provided a bottle if they can support it on their own which, poses/posed a potential health, safety or personal rights risk to children in care.
