Infant care center with 19 infants and 7 teachers in segregated ambulatory and non-ambulatory rooms maintains proper infant sleep protocols with transparent supervision walls. All safety and compliance systems verified operational. One Type B deficiency and two technical violations cited.
Unannounced inspection of infant room discovered rat infestation and structural deterioration. Type A and Type B citations issued for rat droppings in crib area and broken/rotting baseboards.
Complaint investigation substantiated two critical violations: partially used infant formula bottles periodically saved and refed to infants (creating bacterial growth hazard), and infants placed on backs with bottles despite inability to support independently (choking hazard).
Clean annual inspection. All required regulations met with 9 infants properly supervised by 3 staff. Facility within ratio and capacity with proper food handling, infant bedding, medication storage, and emergency preparedness.
The complaint findings determined that unqualified staff had been left alone with infants on more than one occasion. The report was delivered as a Type B deficiency.
The follow-up on a head injury found that the play climber was being used by a child younger than the manufacturer's minimum age recommendation. LPA determined the equipment was intended for children 18 months to 5 years, so use by a younger infant was cited as a violation.
Follow-up case management visit found a service-plan compliance issue after staff offered a child water contrary to the child's needs and services plan. The infant room still had appropriate ratios and capacity.
While conducting an inspection for another matter, LPA observed unsafe infant sleep practices in the infant room. Several infants were napping on boppies on the floor instead of in cribs, and one staff member was not associated to the facility.
