Reporting Requirements - Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by: An incident that happened in October 2024 involving a child who was left unsupervised outside Room 8 was not reported to Licensing. This posed a potential risk to the health, safety, and personal rights to children in care.

Fixtures, Furniture, Equipment and Supplies - A comfortable temperature for children shall be maintained at all times.(1) The licensee shall maintain the temperature in rooms that children occupy between a minimum of 68 degrees F (20 degrees C) and a maximum of 85 degrees F (30 degrees C). This requirement was not met as evidenced by:LPA learned from interviews that the temperature in few of the preschool classrooms were below 68 degrees Fahrenheit during the power outage that occurred on 02/21/23 to 02/22/23.

(1) A ratio of one fully qualified teacher (as specified in Section 101216.1(c)) and one aide for every 18 children in attendance in a preschool program is allowed when the aide meets the qualifications specified in Section 101216.2(d). This requirement was not met as evidenced by: LPA learned from interviews that on 11/22/21 a fully qualified preschool teacher and a teacher's aide who completed more than 6 semester or equivalent quarter units in early childhood education were supervising 21 children in the playground. This poses a potential risk to the health, safety, and personal rights of children in care.
