Child's Records. (a) A separate, complete and current record for each child is maintained in the child care center. This requirement was not met as evidenced by: Based on a file review for child C1 performed on 10/21/25, the facility was unable to provide the LPA with child C1's record during the initial investigation visit. The center director, Anais Heim stated that she was unable to locate the contents of C1's file and that no other records were available for review at the time. This poses a potential risk to persons in care.

(b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). This requirement was not met as evidenced by: Based on an interview with the acting center director, Anais Heim on 4/11/25, it was corroborated that the facility's former center director, Melissa Terhune is no longer employed by this facility effective 03/14/25. The Department has not been notified of this change. This poses a potential risk to persons in care.

Based on a file review performed by the LPA on 08/05/24, the LPA observed thatstaff S3 and S6 did not posess an LIC503 "Health Screening Report" in their personnel file. This poses a potential health, safety or personal rights risk to persons in care.

Based on the LPA's observations on 08/05/24 at 02:15pm, the LPA observed that at least 15 napping preschool children did not have a sheet to cover their cot during nap time. The assistant director, Anais Heim informed the LPA that sheets are available for every napping child at the center, however, staff are not using the sheets on all cots during nap time. This poses a potential health, safety or personal rights risk to persons in care.

Based on a file review performed by the LPA on 08/05/24, the LPA observed that staff S3, S4, and S6 did not have an immunization log in their personnel file showing that he or she has been immunized against pertussis and/or measles. This posed a health, safety and personal rights risk to the children in care.
