Based on record review and interview, the licensee did not meet the requirement stated above. It was determined that a physician's report that was submitted after 6/2022 could not be located today thus the facility could not confirm that the document was safeguarded or being kept confidential as required. This poses a potential health, safety, and or personal rights risks to children in care.

Based on observation and interview the licensee did not meet the requirement stated above. Today, LPA observed children napping without mats or cots and representatives confirmed the observation. This poses a potential risk to the health, safety and or personal rights of children in care.
