Based on observation, the licensee did not comply with the section cited above by having fire extinguisher that was purchased on 6/16/21 and has not been serviced which poses/posed a potential health, safety or personal rights risk to persons in care.

Admission agreements shall be signed and dated, ... no later than seven days following admission. Attachments to the agreement may be utilized as long as they are also signed and dated as prescribed above.This requirement is not met as evidenced by: Based on observation and interview, licensee failed to ensure that a completed Admissions Agreement is placed in the record file of R1.

§1569.625 Staff training; legislative findings; contents : (a)(b)(1)....require staff members of residential care facilities for the elderly who assist residents with personal activities of daily living to receive appropriate training. This training shall consist of 40 hours of training. A staff member......shall be completed within thefirst four weeks of employment. -This requirement is not met as evidenced by:-Based on records review and interview, the licensee did not comply with the section cited above for not having S1 and S2 complete the required hours of training which poses potential health, safety and personal rights risks to persons in care.

87465 Incidental Medical and Dental Care (a) A plan for incidental medical and dental care shall be developed by each facility. The plan shall encourage routine medical and dental care and provide for assistance in obtaining such care, by compliance with the following: (5) The licensee shall assist residents with self-administered medications as needed. This requirement was not met as evidenced by: Based on interview and record review Licensee did not comply with the section cited above. Facility reached out to Resident R1's medical provider when R1 had one pill left which poses an immediate risk to the health and safety of residents in care.
