BUILDINGS AND GROUNDS

Based on LPA observations, the licensee did not comply with the section cited above as the play area was not kept clean, safe, sanitary and in good repair. The sandbox had approximately 4 to 5 inches of standing water accumulated in the potholes. Furthermore, LPA Herrera observed the building’s wall under the play yard’s patio to have mold/algae, and the play structure had grime and debries in the stair’s surface which poses/posed a potential health, safety or personal rights risk to persons in care.

STORAGE SPACE

Based on LPA observations, the licensee did not comply with the section cited above as the facility stored cleaning solutions near food in the facility's pantry. Furthermore, LPA Herrera observed that the facility had fresh produce located next to the sink that is used for cleaning purposes such as mopping. LPA Herrera also observed a hosepipe that was attached to a dispenser that had cleaning solution that was just above the sink near the fresh produce, which poses/posed a potential health, safety or personal rights risk to persons in care.

CHILD'S MEDICAL ASSESSMENTS

Based on record review, the licensee did not comply with the section cited above. Record review indicated that Child #2 did not have proof of tuberculosis test screening/results on file which posed a potential health, safety, or personal rights risk to chidlren in care.

OUTDOOR ACTIVITY SPACE

Based on observation the licensee did not comply with the section cited above in LPA observed preschool play structure with no cushion material to absorb falls which poses a potential health, safety or personal rights risk to persons in care.

FOOD SERVICE

Based on observation, the licensee did not comply with the section cited above. During today's inspection LPA Gibson observed food (chicken nuggets) in a large zip lock bag wide open with no marked date to indicate the content, date of storage or date of expiration which poses a potential health, safety or personal rights risk to persons in care.

DRINKING WATER

Based on observation the licensee did not comply with the section cited above in during today's inspection LPA observed in 3 classrooms no fresh drinking water was readily available tot he daycare children. On this day the preschool children outdoors had no water provided temperature outdoors was 88 degrees with a high of 101 degrees which poses a potential health, safety or personal rights risk to persons in care.

Food Services; (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This requirement was not met as evidenced by: Based on interviews and records review, facility staff served food to Child 1 that is indicated in Child 1’s record that Child 1 has a possible allergy to. This poses an immediate risk to the health, safety, or personal rights of children.

FIXTURES, FURNITURE, EQUIPMENT AND SUPPLIES

Fixtures, Furniture, Equipment and Supplies; (m) All play equipment and materials used by children shall be age-appropriate. This requirement was not met as evidenced by:Based on observation and interview, Licensee failed to provide age-appropriate play equipment and material by allowing two-year-old children to play with “play foam” that is intended for children age three and older and can be a choking hazard. This poses an immediate risk to the health, safety, or personal rights of children.

ARTICLE 2. Administration of Child Day Care Licensing

On or before March 30, 2018... a person who... is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training... and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not met as evidenced by: Based on records review 11 out of 17 staff files did not contain a current mandated reporter training. This poses a potential risk to the health, safety, or personal rights of children.
