Graham Family Day Care

Annual inspection identified missing required forms, fire extinguisher overdue for service (last serviced 9/20/2023), unsecured fireplace (technical violation), and staff/child immunization and Mandated Reporter training deficiencies.

The home generally operated safely and within ratio, but infant sleep supervision and child file completeness were problems. The inspector also confirmed the home had proper fire safety equipment and documented CPR/first aid training.

Complaint investigation substantiated two personal rights allegations: staff hitting children and staff covering an infant's mouth. Child and adult interviews described repeated head slaps or mouth-covering behavior when children cried or were upset.

This case-management visit documented the home being out of ratio for 25 minutes and an infant sleeping in a rocker instead of approved sleep equipment. The issue was observed while children were napping and the assistant was alone with the group.

Complaint investigation was substantiated for physical discipline against daycare children. The licensee admitted to using discipline that included striking children in the past, and disclosures from witnesses aligned with those allegations.
