Follow-up case management visit regarding incident where child was unsupervised for approximately 3 minutes when they entered unlocked classroom on 3/10/25. Facility implemented corrective measures including relocating office desk for better visibility and locking playground door when children outside.

Case Management inspection of Title 5 funded program on inactive status. Facility began operation 08/21/24 for new school year. Site Supervisor missing required preventative health and safety training verification.

Serious incident investigation: child was restrained in bathroom by staff on two separate dates (11/16/23 and 2/14/24). School board investigation began December 2023 but failed to notify Department until later, incorrectly treating it as K-6 rather than preschool issue.
