    Based on observation, interview and record review the licensee did not comply with the section cited above in which 3 childrens records (C1 - C3) did not have written approval or instructions from the child's authorized representative on when and how to administer medications which poses/posed a potential health, safety or personal rights risk to persons in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time …Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interviews the licensee failed to ensure staff were providing visual supervision of the children resulting in a child's injury. This poses an immediate Health, Safety or Personal Rights risk to the children in care.

    Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861.This requirement is not met as evidenced by: Based on LPA's observation of several brochures and business card the facility used to advertise with the intent of attracting client(s), however; the brochures and cards did not contain the facility number. This poses a potential health, safety and/or personal rights risk to the children in care.

    The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance.This requirement was not met as evidenced by: Based on LPA's observation of S1 & S2 left alone to supervise 21 children at 9:27am. This poses/posed a potential health, safety and/or personal rights risk to the children in care.

    101215.1(b) All child care centers shall have a director.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as they have been operating without a Director which poses a potential health, safety or personal rights risk to persons in care.

    101212(d)… during the operation of the childcare center of any of the events… a report shall be made to the Department by telephone or fax within the next working day and during its normal business hours. This requirement is not met as evidenced by:Based on interviews and facility record review, the center has not notified and submitted any Unusual Incident Report’s to CCL in the past six months when incidents did in fact occur.
