Case management incident investigation found that child C1 walked away from supervised playground and was left unattended on a ramp to Classroom #2 for approximately 2-4 minutes while staff assisted a parent with sign-out procedure.

Annual inspection found the preschool largely compliant on health, safety, and records, but multiple staff had expired mandated reporter training and the director lacked lead training certification. Overall safety conditions, ratios, and child records were otherwise in good order.
