Three out of six staff stated they have they have left children alone inside the classroom when they are assisting another child inside the restroom or changing a child. One staff stated they have witnessed a child being left on the play yard alone which poses an immediate health, safety or personal rights risk to persons in care.

LPA interviewed six staff. Three out of six staff stated they have been out of ratio. Four out of six staff interviewed stated they have witnessed other staff being out of ratio. On 9/12/2024, LPA reviewed records where a teacher had informed the front office staff they were out of ratio. One out of the three parents interviewed stated they have observed one staff caring for 13 to 14 children alone in the morning during drop-off which poses an immediate health, safety or personal rights risk to persons in care.

101218.1 Admission Procedures and Parental and Authorized Representative's Rights(a) In accordance with the child care center's individual program, policies and needs, the licensee shall develop, implement and maintain an admission procedure that enables the person in charge of admissions to: (B) Provides the child's parent or authorized representative with information about the child care center that shall at least include the child care center's admission policies and procedures, activities, services, regulations, hours and days of operation, fees, procedures to be followed should the child become ill or injured while at the child care center, and procedures for conducting inspections for illness. Based on LPA interview with Director who stated Accident/Injury reports were not physically given to the parent of Child (C1) and copies of Accident/Injury reports with the director and parent signatures missing.

The licensing report provided by the department shall be posted immediately upon receipt, adjacent to the postings required pursuant to Section 1596.817 and on, or immediately adjacent to, the interior side of the main door to the facility and shall remain posted for 30 consecutive days. On 3/25/22, LPA upon entrance into the facility observed the Notice of site visit was not posted by the facility representative issued on 3/18/2022. Civil penalty is being assessed for failure to post the Notice of Site visit.This is a potential risk to Health and Safety to the children in care.
