    Based on record review, the licensee did not comply with the section cited above. Staff 1 most recent tuberculosis test was given in 2001, proof of tuberculosis current test results or proof of chest x-ray results are not available for review during this visit, which poses a potential health, safety or personal rights risk to persons in care.

Per Directors own admission, they were not aware the 15-minute check should be documented and maintained for infants 12 months - 24 months. This poses/posed a potential health, safety or personal rights risk to persons in care.

In reviewing staff records, it was determined that Staff #1, #2, and #4, did not have proof of the required immunizations on file.     Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA determined that several needs and services plan were not updated. This poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above. LPA observed two infants in sleep sacks, sleeping in their assigned cribs. Although not swaddled, sacks still restricted the movement. This poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation and interview, the licensee did not comply with the section cited above. Based on previous visit(s), photos taken, today's observations, and staff disclosure, the partitions that separate the sleeping area from the activity space, have been moved.     Based on record review, the licensee did not comply with the section cited above. In reviewing staff records it was determined that Staff #1, #4 and Staff #7, did not have current proof of completion for the required training. This poses/posed a potential health, safety or personal rights risk to persons in care.

  • PERSONNEL REQUIREMENTS

    Based on record review, the licensee did not comply with the section cited above. In reviewing staff files, LPA determined that Staff #1 and #6, did not have the required health screening/TB test on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

  • 101438.3(b)(2)Type B

    Indoor Activity Space for Infants - Moveable walls or partitions, if used, shall be at least four feet high; shall be constructed of sound-absorbing material; and shall be designed to minimize the risk of injury to infants. This requirement was not being met as evidenced by observation and measurement taken of the partitions being less than four feet high. This poses/posed a potential health, safety risk to the children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

    Responsibility for Providing Care and Supervision for Infants: Each infant shall be constantly supervised and under direct visual observation and supervision by a staff person at all times. This requirement was not being met as evidenced by interview(s) and document review. A child suffered cuts and abrasions while in care however, only assumptions were made as to what may have happened but, all staff that were providing care and supervision to the child, on the day in question, did not know what happened.
