Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.This requirement was not met as evidenced by: Based on interviews and a review of a photograph, it was determined that facility staff failed to ensure a child’s personal rights were met. Staff were aware a child was wearing pants with a soiled crotch and no undergarments yet returned the child to care without addressing the child’s toileting and hygiene needs. This is poses a potential health, safety and personal risk to children in care.

Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interviews and Staff's own admission, the licensee did not comply with the section cited above in that it was determined that on 1/16/25 there was an Absence of Supervision resulting in C1 was left unsupervised, which poses an immediate health and safety risk for persons in care.
