Complaint investigation substantiated; Teacher 1 failed to inform parents or site supervisor in timely manner of an incident.

Incident investigation following Unusual Incident Report filed for event on 8/5/2024. Staff member forcefully held child down on cot and hit child on head multiple times. Type A citation issued for immediate risk to health and safety. Witness statement and child account confirmed incident.

Type A violation: Child left unattended outside during transition, wandered into empty 4-year-old classroom and remained unsupervised for approximately 10 minutes. Facility failed to provide adequate supervision.

Complaint investigation substantiated allegations of inappropriate physical discipline. Staff #5 swept feet of child to force sitting; Staff #7 pinched one child and struck another with a toy block.

LPA conducted a case management inspection to address several incidents that had been reported late to Community Care Licensing. Based on interviews and the director's admission, the facility was cited for failing to report incidents involving personal rights violations and a child left unattended within the required timeframe.

Preschool annual inspection found overall compliance with minor maintenance recommendations. Four of ten children files missing physician reports and TB assessments. All staff mandated reporter training outdated but facility stated completing renewal.
