    CHILD'S MEDICAL ASSESSMENTS

    Based on record review, LPA observed that 0 of the 6 children reviewed had not completed a medical assessment. All children reviewed were missing the LIC 701 Physician's Report in their file. Child #1 and Child #5 were also missing an immunization record on file. the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

  • OUTDOOR ACTIVITY SPACE

    Based on observation of the yard, LPA observed there to be a gate that was open to an area off limits area. The gate does is currently unable to close properly. LPA also observed two long white tables that broken in multiple areas and a broken spiderman chair desk. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

    101161 Limitations on Capacity(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement was not met as evidenced by: interviews conducted and record review. It was revealed that the facility is providing care to a school age child (7 years old) without a school age license. This poses an immediate health, safety and/or personal rights risk to children in care.

    (1) A licensed child day care center, as defined in Section 1596.76,...shall have its drinking water tested for lead contamination levels...no later than January 1, 2023, and every five years after the date of the initial test.This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above. Licensee failed to provide test results for the building as the building was constructed before January 1, 2010. This poses a potential health, safety, or personal rights risk to children in care.

    Based on interview and record review, the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care. Per licensee, the facility has not been tested for lead. Licensee stated that the children do not drink from the drinking fountains and only drink bottled water that they bring from home. Licensee did stated that she uses one of the sinks for boiling water when cooking.

  • CRIMINAL RECORD CLEARANCE

    (e) All individuals subject to a criminal record review...shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department. This requirement was not met as evidenced by: record review and interview. Information obtained from the interviews. During interviews Adult #1 was identified as being a staff member at the facility. A review of the facility fingerprint clearance roster shows that Adult #1 was not fingerprint cleared at that time. This is an immediate risk to the health, safety or personal rights of children in care.

    BUILDINGS AND GROUNDS

    Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by LPA observation. During today’s inspection LPA observed outside a big wheel toy with both pedals missing. LPA also observed a door with a missing door knob. The missing items caused pointed parts to be exposed. This is a potential risk to the health, safety or personal rights of children in care.

    Current roster of children provided care in facility required. Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request. This requirement was not met as evidenced by LPA observation. LPA did not observe a children's roster. Director stated she has not kept one. This is a potential risk to the health, safety or personal rights of children in care.
