Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed Teacher #1 supervising five infants alone in one room which poses a potential health, safety or personal rights risk to persons in care.

OUTDOOR ACTIVITY SPACE
Based on observation, the licensee did not comply with the section cited above. LPA observed rotten persimmons in the grass area of the infant outdoor activity space. There were spider webs and dirt covering a play house and a play structure. LPA also observed an unrolled water hose in the outdoor activity space which is a tripping hazard. This poses a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on observation, interviews, and records review, the facility did not notify the Department that infants had to be moved to an alternate classroom or report that repairs were needed due to rodents entering a classroom which poses a potential Health and Safety Risk to persons in care.

This requirement was not met as evidenced by: Based on observation, interviews, and records review, rodents entered the infant center in several rooms. This posed a potential Health and Safety Risk to persons in care.
