    Based on observation, the licensee did not comply with the section cited above. During today's inspection, LPA observed Teacher #1 supervising five infants alone in one room which poses a potential health, safety or personal rights risk to persons in care.

  • OUTDOOR ACTIVITY SPACE

    Based on observation, the licensee did not comply with the section cited above. LPA observed rotten persimmons in the grass area of the infant outdoor activity space. There were spider webs and dirt covering a play house and a play structure. LPA also observed an unrolled water hose in the outdoor activity space which is a tripping hazard. This poses a potential health, safety or personal rights risk to persons in care.

    101212 Reporting Requirements (d)... a report shall be made to the Department... (1) Events reported shall include:... (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by: Based on observation, interviews, and records review, the facility did not notify the Department that infants had to be moved to an alternate classroom or report that repairs were needed due to rodents entering a classroom which poses a potential Health and Safety Risk to persons in care.

    101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary... to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met as evidenced by: Based on observation, interviews, and records review, rodents entered the infant center in several rooms. This posed a potential Health and Safety Risk to persons in care.

    Responsibility for Providing Care and Supervision - The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement is not met as evidenced by interviews with witnesses, records review conducted during today’s inspection. Staff unknowingly left child in play yard for less than one minute. This poses an potential risk to the health, safety or personal rights of children in care.

  • REPORTING REQUIREMENTS

    Reporting Requirements - Upon the occurrence, during the operation of the child care center... a report shall be made to the Department by telephone or fax within the Department's next working day ... In addition, a written report... shall be submitted to the Department within seven days following the occurrence of such event. This requirement is not met as evidenced by interviews with witnesses conducted during complaint investigation. Agency Administration directed staff not to report to Fresno Regional Office two (2) separate incidents which a child was found alone outside of the classroom AND child was injured and required medical treatment. This poses an potential risk to the health, safety or personal rights of children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Responsibility for Providing Care and Supervision - No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by interviews with witnesses, conducted during recent complaint investigation. Interviews indicated staff found the child alone and unsupervised in an off-limits area for an undetermined amount of time. This poses an immediate risk to the health, safety or personal rights of children in care.
