No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Child#1 sustained an injury at the facility due to being near the front entrance door which is accessible to children during outside play time. This is a potential risk to children in care.

A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. This requirement was not met as evidenced by the fact that Staff#1 was left alone with 15 napping children and 3 children sitting on the table for about 10 minutes before all 3 children went to lay down for their naps. This is a potential risk to children in care.
