This requirement was not met as evidenced by: Based on records review and confidential interviews, the CCC’s roof had a leak causing the tile(s)in Room #2 to bulge, leak and be discolored. The CCC continued to utilize the room and have children and visitors present.

This requirement was not met as evidenced by: Based on observation and confidential interview with director it was confirmed that toilet closest to the wall in the bathroom between Rooms #4 & #2 does not flush.
