This requirement was not being met as evidenced by: Based on interview, S2 admitted to forcefully removing C1 from the window by their arm.     101238(a) Building and Grounds (a) The childcare center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by: Based on records review and confidential interviews, the CCC’s roof had a leak causing the tile(s)in Room #2 to bulge, leak and be discolored. The CCC continued to utilize the room and have children and visitors present.

    101239( e)(4) Fixtures, Furniture, Equipment and Supplies ( e) Faucets used by children for personal care shall deliver hot water. (4) All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition...conveniences shall be provided...in centers that serve children with physical disabilities. This requirement was not met as evidenced by: Based on observation and confidential interview with director it was confirmed that toilet closest to the wall in the bathroom between Rooms #4 & #2 does not flush.
