Hazardous items were accessible to children during inspection: adult scissors, sunscreen containers, and cleaning solution-containing mop located in childcare areas. All items were removed by staff during the visit.

The follow-up focused on a child who entered the building unsupervised from the playground for about one to two minutes before being noticed. Video review showed the supervising teacher missed the child's movement during a parent exit, and the repeated supervision failure resulted in a Type A citation.

During a case management incident review, child #1 was left unattended on the preschool/pre-K playground for about 9 minutes during a transition indoors. The attendance log used for transitions was not used, and the facility was cited for a Type A deficiency.

A complaint investigation reviewed surveillance video and confirmed a sanitary-condition issue in the classroom. The analyst observed a deceased rodent on a ceiling tile when workmen removed it, and the allegation was substantiated.

Case management inspection found a child had been left unsupervised in the outdoor play yard long enough to be injured after the gate was left open. The gate latch had since been repaired, and the facility conducted staff retraining on supervision after the incident.
