Plan of Correction follow-up visit revealed new Type A violations. Facility failed to provide acknowledgement forms to newly enrolled children's parents and failed to correct prior deficiency about site director presence.

Plan of Correction follow-up found partial compliance with previous deficiency citations. Some violations were corrected but critical documentation for site director designation was not received, resulting in failure to correct with civil penalties.

Case management-deficiencies inspection revealed critical violations. No director on-site (Type A citation). Teacher aides leading classrooms without qualified teacher supervision (Type B citation). Classroom with 31 school-age children supervised by only 2 teacher aides—repeated violation (Type A with civil penalties).

Type A violation cited for operating over capacity. 31 school-age children in Room 1 with only 2 staff members creating immediate risk to health and safety. Civil penalties and parent notification requirements apply.

School age program annual inspection. All safety features operational. Type B citation issued for failure to complete lead water testing by January 1, 2023 deadline.

Complaint investigation with mixed findings. While some allegations were unsubstantiated, at least one allegation was found SUBSTANTIATED with deficiency citations issued per Title 22, Division 12 California Code of Regulations.

New director approved by licensing. One staff member lacked fingerprint clearance (Type A violation). Facility maintained appropriate supervision ratios with all required documentation posted and staff records complete.
