This requirement was not met as evidenced by: Based on interviews conducted by the LPA with staff S2 and S4 on 04/11/25 and the Unusual Incident Report submitted by the facility on 03/26/25, it was corroborated that on 03/25/25 at approximately 11:25am, child C1 was left without supervision of a teacher for approximately eight minutes while on the center's two's play yard before being reunited with their class. This presents a potential threat to the health and safety of children in care.

All licensees shall ensure the inaccessibility of pools, including swimming pools (in-ground and above-ground), fixed-in-place wading pools, hot tubs, spas, fish ponds or similar bodies of water, through a pool cover or by surrounding the pool with a fence.This requirement was not met as evidenced by: Based on the LPA's observations on 01/07/25 at 11:15am, during an outdoor play yard walkthrough with Director Shafer and Assistant Director Petersen, the LPAs observed building materials, children’s play equipment (sand and water tables), and other plastic play furniture not intended for water use that have accumulated water at least several inches deep.This poses an immediate health and safety risk to the children in care.

This requirement was not met as evidenced by:Based on the LPA's interviews with facility staff and the center director Daysi Lemus’ own admission on 09/19/24, it was corroborated that the facility operated out of ratio on a specified date resulting in one staff member (S2) supervising 14 awake children. This posed a potential health and safety risk to the children in care.

This presents a potential threat to the health and safety of children in care.

Reporting requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by: Based on the LPA's interviews, the center director, Jamie Poma admitted that incidents of staff S7 roughly handling children in care were not reported to the licensing agency as required by regulations. This posed a potential health and safety risk to the children in care.
