    (a) The licensee shall ensure that each child is accorded the following personal rights:(2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Based on interviews and documents collected the above mentioned regulation was not met, C#2 dispalyed agressive behaviour towards other children in care. There were multiple incidents, which includes a 2/22/25 incident where C#2 kicked C#1 in the face one report stated it was accdientally another report stated it was not accidentally.

    (m) All play equipment and materials used by children shall be age-appropriate.record review, revealed this requirement was not met: On 11/18/2024 at 10:12AM during outdoor play, C1 was walking on the obstacle course when C1 lost C1's balance and hit C1's head on a wooden log. It caused bleeding and abrasion the the right side of the forehead. 911 was contacted and C1's abrasion required stiches. Facility could not provide manufacturer instructions for all materials(wooden logs) that c1 interacted with the day of the incident.

    (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision ....this regulaiton was not met .. Due to incident that occured on 7/30/2024, when S#1 transitioioned 8 children fromt the classroom to the preschool play ground, C#1 accidently leaft classroom#2, and C#2 was left outside during transitioning with out care and supervision.

This requirement was not met as evidenced by: LPA observed 2 children behind storage units without teacher supervision. Based on LPA observations, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
