This requirement was not met as evidenced by: 4/30/2025 at 1:45 PM LPA's observed 2 of 3 toilets in the preschool classroom were out of order clogged by waste and other debris, and 2 of the sinks inside the bathroom had no running water which poses a potential health and safety risk to children in care.

S1 admitted this incident occurred. This resulted in bruises on C1’s arm and made child upset, cry. This is a violation of personal rights and potential risk to safety of children. Since action was unintentional Type B is cited .
