    101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by: 4/30/2025 at 1:45 PM LPA's observed 2 of 3 toilets in the preschool classroom were out of order clogged by waste and other debris, and 2 of the sinks inside the bathroom had no running water which poses a potential health and safety risk to children in care.

    101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature [...] This requirement is not met as evidenced by: It was determined staff S1 was helping child C1 during bathroom break, she held/squeezed C1’s upper arms to stop C1 from sitting on a soiled pull-up and have an accident on the floor. S1 admitted this incident occurred. This resulted in bruises on C1’s arm and made child upset, cry. This is a violation of personal rights and potential risk to safety of children. Since action was unintentional Type B is cited .
