Navigate to Facility Intel in the sidebar under Intelligence. Your report shows:- Days since last visit — how long since your last inspection
- Similar facilities — how many comparable facilities are tracked
- Risk level — overall assessment with active factor count
- 6-factor table — each risk factor with status and details
Risk levels
| Level | What it means |
|---|---|
| All clear | Low risk. Keep doing what you’re doing. |
| Monitor | Some factors need attention but nothing urgent. |
| Elevated | Multiple risk factors active. Review recommendations. |
| Urgent | High risk of inspection. Take action this week. |

How long since your last inspection compared to similar facilities. When ACTIVE, you’re overdue compared to most facilities — expect an inspection soon.
Hot zone
Hot zone
Whether inspectors have been visiting nearby facilities recently. When ACTIVE, inspectors visited facilities near you in the last 14 days — your turn may be coming.
Citation pattern
Citation pattern
Your recent citation history. ACTIVE means citations in the last 90 days. Review what was cited and ensure corrections are documented.
Cluster risk
Cluster risk
How many overdue facilities are near you. Regional offices often schedule sweep visits when multiple facilities in an area are overdue.
RO activity spike
RO activity spike
Whether your regional office is conducting more inspections than usual. This often happens at quarter-end or when new inspectors are assigned.
Complaint flag
Complaint flag
Whether complaints have been filed against your facility in the last 180 days. Complaints can trigger priority inspections.
